WHAT IS A HEALTHCARE PROXY (AKA MEDICAL POWER OF ATTORNEY)?

A Healthcare Proxy allows you (the “Principal”) to appoint a third party (the “Agent”) to make healthcare decisions on your behalf if you are in a state where you are unable to make a decision for yourself.

HOW MUCH IS A HEALTHCARE PROXY?

We charge $250 for a Healthcare Proxy.

HEALTHCARE PROXY V LIVING WILL

Living Will allows you to express your wishes regarding medical care and end of life treatment decisions.  A Healthcare Proxy on the other hand appoints an individual to make those decisions for you.

HEALTHCARE PROXY V. POWER OF ATTORNEY

Financial Power of Attorney allows an agent to conduct financial business on behalf of the Principal.  A Healthcare Proxy allows an agent to make healthcare decisions on behalf of the Principal when the Principal cannot make those decisions him or herself.

We offer a Power of Attorney Package (which includes both a Financial and Medical Power of Attorney (aka Healthcare Proxy)) for $400.

DO I NEED AN ATTORNEY TO DRAFT A HEALTHCARE PROXY?

Legally, you do not need an attorney to draft a healthcare proxy.  However, in order for a Healthcare Proxy to be legally valid, it has to follow the exact specifications of the law. Failure to follow the exact statutory requirements will result in the healthcare proxy not being accepted.
