An Advance Directive (also known as a Living Will) specifies your desires to doctors and hospitals regarding medical treatments and end of life procedures in situations when you are unable to make decisions on your own.

We charge $375 for an Advance Directive.
ADVANCE DIRECTIVE V HEALTHCARE PROXY
An Advance Directive allows you to express your wishes regarding medical care and end of life treatment decisions. A Healthcare Proxy on the other hand appoints an individual to make those decisions for you.
ADVANCE DIRECTVE V LAST WILL
A Last Will allows you to express your desires regarding the distribution of assets and appointment of guardians for dependents and pets if you pass away. An Advance Directive allows you to express your desires regarding medical care and end of life treatment decisions while you are alive, but unable to make the decision on your own due to physical or mental incapacity.
DO I NEED AN ATTORNEY TO DRAFT AN ADVANCE DIRECTIVE?
Legally, no. However, in order for an Advance Directive to be legally valid, it has to follow the exact specifications of the law. Failure to follow the exact statutory requirements will result in doctors and hospitals not accepting the Advance Directive.
